TIER 1 – (First Available/Sick visits, Primary Care visits)

TIER 2 – (Tier 1 visit + injection, Initial Doc visit, Pediatric Well Child)

Member: $60.00

Non-Member: $249.00

TIER 3 – (Any procedure, Well Woman’s Exam)

Member: $125.00

Non-Member: $349.00

Specialty Services

  • Functional Medicine $499 ($125 for Members)

  • Cognitive Assessment and Treatment $499 ($125 for Members)

  • I.V. Infusion - variable as you can build your own - start at $150.00 (10% off for members)

  • EKG - $125.00

  • Weight Loss Consultation $315 (10% off for members)

* X-Rays are performed by a contracted company and are paid separately from doc.  These cost $125.00.
